Nurse's Corner

Parents of Kindergarten Students: 

PLEASE BE SURE YOU HAVE SUBMITTED ALL HEALTH DOCUMENTS FOR SCHOOL ADMISSION  (Documentation of a physical exam done within the past two years, complete immunization record, date of blood lead level, and a Tuberculosis Risk Assessment or proof of a Mantoux skin test).

Also please send a complete change of clothing for your student, including shoes and socks.  The nurse's office may not have extra clothing to give your student in case of a spill or accident.

Parents of All Students:

EACH STUDENT MUST HAVE AN EMERGENCY CARD ON FILE IN THE NURSE'S OFFICE!  Cards are to completed and returned to your student's teacher by Friday, September 6, 2019.

PLEASE NOTIFY THE SCHOOL NURSE IF YOUR STUDENT HAS A CHRONIC CONDITION (asthma, allergies, diabetes, etc.).
